Notetaker Request Form

If you are requesting a notetaker, please be aware of the following: 
Only students that are registered with Student Disability Services (SDS), have a disability that affects their ability to take notes, and have been approved by SDS for notetaking accommodations are eligible for notetaking services. 

Before being approved for notes in a particular class, you must first request you accommodation letter for that class.

Please include both subject code and 4-digit course number (e.g., PSYCH 1101)
Numbers only, please. (e.g., 001)
Numbers only, please. (e.g., 401, 201. Leave blank if none)

Please note: Notes will be delivered through SDS Online Services, all students will receive notes anonymously. Notetakers will not know the name of students receiving notes.

Submission of this form does not guarantee your request will be approved. After submission of this form, either your SDS counselor or the SDS Notetaker Coordinator will follow up with you directly.
